High-touch, deeply personalized behavioral health case management for individuals and families navigating complex circumstances. Built around the real-world challenges facing those in early recovery.
More than case management. A system for building a life.
The CORE Case Management Program is a high-touch, deeply personalized service built around the real-world challenges facing those in early recovery. While the team retains the capacity to manage high-acuity psychiatric cases, substance use recovery, and complex behavioral issues, the true specialty lies in helping clients build a life worth living from the ground up.
This is accomplished by integrating structure, strategy, and a deeply clinical lens with personal connection and purpose-driven support. Every plan is individualized. Every outcome is client-centered. The goal is to not only stabilize clients but to activate them: to help them take ownership, define their goals, build skills, and ultimately carry the weight of their own lives.
Unlike many "coaching" or standard case management models, our team includes clinically trained and licensed professionals at the master's level. They bring lived experience, professional rigor, and evidence-informed approaches into the trenches of each client's life.
"Clients don't just talk about goals. They learn how to define them, challenge the thoughts and behaviors that block them, and take measurable action."
When families are tired, clients are stuck, and there's disconnect in care and continuity, this is where we come in. A surviving attachment to rely on and support growth. The service is grounded in evidence-informed principles of Mindfulness Based Cognitive Behavioral approaches (MCBT).
Every interaction is an opportunity to realign with what the client wants and interrupt the cycles that have kept them stuck. It's structured, intentional change work, and it's the reason this model actually moves people forward.

Family as Foundation
The family system is central to the work.
At Coast Health, we view family involvement not as optional but as fundamental. Whether the family is actively engaged or estranged, functional or fractured, it remains the primary relational ecosystem for most clients.
As such, we directly engage families as co-architects in the process. This may involve mediation, psychoeducation, boundary-setting, crisis de-escalation, or collaborative goal tracking. Our case managers help recalibrate family systems to support, not sabotage, progress.
This work is structured, contract-bound, and clinically informed. For clients who need more intensive daily support, Companion Services complement case management with structured, on-the-ground oversight.
Clinical Foundation
Mindfulness Based Cognitive Behavioral Therapy
"You are what you think."
The service is grounded in evidence-informed principles of Mindfulness Based Cognitive Behavioral approaches (MCBT). Mindfulness-based cognitive behavioral strategies are delivered by licensed clinicians or incorporated into care planning in coordination with outside providers.
Clients don't just talk about goals. They learn how to define them, challenge the thoughts and behaviors that block them, and take measurable action. We reinforce this through weekly planning, daily accountability, and consistent reflection.
Every interaction is an opportunity to realign with what they want and interrupt the cycles that have kept them stuck. It's not just support. It's structured, intentional change work, and it's the reason this model actually moves people forward.
Frequently Asked Questions
Common Questions
What is behavioral health case management?
Behavioral health case management is independent clinical oversight and structured support for individuals and families navigating mental health or addiction treatment. A case manager serves as your advocate, coordinating between treatment providers, family members, and other stakeholders to ensure continuity of care, appropriate level of care placement, and accountability throughout the recovery journey.
How is independent case management different from facility-based case management?
Independent case managers work exclusively for you and your family, not for a treatment facility. This means unbiased recommendations, objective assessment of treatment progress, and advocacy focused solely on your loved one's best interests. Facility-based case managers are employed by the treatment center and may have conflicts of interest regarding discharge planning or level of care recommendations.
What does a behavioral health case manager do?
A behavioral health case manager provides clinical oversight, treatment coordination, family advocacy, crisis intervention, discharge planning, and ongoing monitoring throughout the treatment journey. They coordinate communication between treatment providers, insurance companies, and family members, ensure appropriate level of care placement, monitor treatment progress, and provide guidance during transitions between levels of care.
How much does private case management cost?
Private behavioral health case management typically ranges from $10,000 to $15,000 per month depending on the level of involvement, complexity of the case, and geographic location. This investment provides 24/7 clinical oversight, treatment coordination, family advocacy, and expert guidance throughout the recovery journey. Many families find that independent case management saves money in the long run by preventing inappropriate placements and ensuring efficient use of treatment resources.
When should I hire a behavioral health case manager?
Consider hiring a case manager when navigating complex treatment decisions, transitioning between levels of care, dealing with multiple diagnoses or co-occurring disorders, experiencing repeated treatment failures, or when you need an independent advocate to coordinate care across multiple providers. Case management is particularly valuable for families seeking high-net-worth, discreet support or those dealing with treatment-resistant conditions.
Do case managers work with insurance companies?
Yes, experienced case managers can coordinate with insurance companies to verify benefits, advocate for appropriate level of care authorizations, and navigate appeals processes when necessary. However, private case management services themselves are typically not covered by insurance and are paid out-of-pocket by families seeking independent, unbiased advocacy.
Can a case manager help with intervention planning?
Yes, many behavioral health case managers are trained in intervention methodologies and can coordinate professional interventions as part of comprehensive case management services. This includes pre-intervention planning, family preparation, coordination with intervention specialists, and immediate post-intervention structured support to ensure seamless transition to treatment.
How long does case management typically last?
Case management duration varies based on individual needs and treatment complexity. Some families engage case managers for 3-6 months during acute treatment phases, while others maintain ongoing relationships for 12-24 months or longer to support sustained recovery and prevent relapse. The most effective case management relationships are flexible and adapt to changing needs over time.
Is CORE Case Management the right fit?
The program is designed for families who require expert clinical oversight and structured support, value discretion and personalized attention, and seek long-term support beyond traditional care timelines. A confidential conversation is the best way to find out.
